If you have been in a car accident and were told at the emergency room that your x-rays came back normal, you may have left thinking you were not seriously hurt. That is one of the most common and most consequential misunderstandings I encounter in my Greenville chiropractic practice.


Soft Tissue Injuries After a Car Accident: What You Need to Know

Normal x-rays do not mean no injury. They mean no broken bones or obvious structural damage visible on standard imaging. The soft tissue injuries that are responsible for the majority of pain and dysfunction following a car accident, the muscles, tendons, ligaments, and discs that absorb the force of a collision, are largely invisible on standard x-rays. They require a clinical examination by someone trained to identify them, and they require treatment that addresses what actually happened to those structures.


After more than 40 years of treating auto accident patients in Greenville, soft tissue injuries are what I spend most of my time diagnosing and treating. Here is what you need to know about them.


What Are Soft Tissue Injuries?


Soft tissue is a broad term that refers to the muscles, tendons, ligaments, fascia, nerves, and blood vessels that support and connect the skeletal system. In the context of auto accident injuries, the soft tissues most commonly affected are those of the cervical and lumbar spine, the surrounding musculature, and the discs between the vertebrae.


When a car accident subjects the body to sudden, violent forces, these structures are stretched, compressed, torn, and in some cases ruptured in ways that can produce significant pain, inflammation, and functional limitation. The severity of the injury does not always correlate with the speed of the collision or the amount of vehicle damage. Low-speed rear-end collisions are among the most common causes of significant soft tissue injuries because of the way the force transfers through the vehicle to the occupant's spine.


The Most Common Soft Tissue Injuries After a Car Accident


Muscle Strains

A muscle strain occurs when muscle fibers are overstretched or torn. In auto accident injuries, muscle strains most commonly affect the cervical and lumbar musculature, producing pain, stiffness, spasm, and reduced range of motion. Mild strains involving a small number of torn fibers typically heal within a few weeks with appropriate treatment. More significant strains involving extensive fiber damage can produce symptoms for months and may contribute to chronic pain if not properly addressed.


Ligament Sprains

A ligament sprain occurs when the ligaments that connect the bones of the spine are overstretched or torn. Ligament sprains are a primary component of whiplash injuries and are significant because ligaments have a relatively poor blood supply compared to muscles, which means they heal more slowly and are more prone to incomplete healing if not properly supported during recovery.


Significant ligament sprains in the cervical spine can affect the stability of the vertebral joints, which is one reason why whiplash that is not properly treated can lead to long-term cervical instability, chronic pain, and accelerated degenerative changes over time.


Disc Injuries

The intervertebral discs are the cushioning structures between the vertebrae and are among the most commonly injured soft tissues in auto accidents. The sudden compressive and shearing forces of a collision can cause the disc to bulge beyond its normal boundary, herniate through the outer annular wall, or in severe cases rupture entirely.


Disc injuries are significant because a displaced or herniated disc can compress the surrounding nerve roots, producing the radiating pain, numbness and tingling, and muscle weakness that patients often experience in the arms or legs following a car accident. Disc injuries are not visible on standard x-rays and require MRI or CT imaging to confirm, though a thorough clinical examination can identify the signs of disc involvement without imaging.


Tendon Injuries

Tendons connect muscles to bones and can be strained or torn in auto accident trauma, particularly in the shoulder, elbow, and other extremity joints when a driver or passenger braces for impact. Tendon injuries produce localized pain, weakness, and difficulty with specific movements and can be slow to heal due to the relatively limited blood supply to tendon tissue.


Fascia and Connective Tissue Injuries

The fascia is the connective tissue that surrounds and connects muscles and other soft tissue structures throughout the body. Fascial disruption from auto accident trauma can produce widespread pain patterns that do not follow the typical nerve distribution patterns, making them harder to identify and treat. Myofascial disruption is a recognized component of auto accident injury and one that responds well to specific manual therapy techniques.


Why Soft Tissue Injuries Are Frequently Missed


There are several reasons why soft tissue injuries from car accidents are so frequently underdiagnosed and undertreated, and understanding them can help you advocate for yourself.


Standard X-Rays Do Not Show Soft Tissue

As I mentioned at the outset, standard x-rays show bones. They do not show muscles, ligaments, tendons, or discs. An emergency room evaluation that relies primarily on x-ray imaging is equipped to rule out fractures and gross structural damage but is not equipped to evaluate the soft tissue injuries that are most common in auto accidents.


This is not a criticism of emergency medicine. Emergency rooms serve a critical function in ruling out life-threatening injuries. But an ER clearance is not the same as a soft tissue evaluation, and many patients leave the emergency room with what they believe is a clean bill of health only to develop significant symptoms in the days that follow.


Symptoms Are Often Delayed

The adrenaline response that follows a car accident temporarily suppresses pain perception. Combined with the inflammatory process that takes time to develop after soft tissue trauma, many patients feel relatively okay in the immediate aftermath of a collision and assume they were not seriously hurt. By the time pain, stiffness, and other symptoms become fully apparent, often 24 to 72 hours after the accident, the injury has already been present and progressing for days.


This delayed symptom onset is one of the most important reasons to get evaluated promptly after an accident even when you feel fine. Waiting for symptoms to develop before seeking care means starting treatment later than optimal and giving the injury time to progress before it is addressed.


Soft Tissue Injuries Are Invisible to Skeptics

It is worth acknowledging that soft tissue injuries are sometimes met with skepticism by insurance companies, adjusters, and even some medical providers, precisely because they do not produce findings on standard imaging. This skepticism is not medically justified. Soft tissue injuries are real, they are documented extensively in the medical literature, and they can produce significant and lasting pain and dysfunction. But it does make thorough clinical documentation from an experienced provider especially important for patients who are pursuing a personal injury claim.


How We Diagnose Soft Tissue Injuries at Brown Chiropractic Center


A thorough clinical examination is the foundation of soft tissue injury diagnosis. When you come in after a car accident, Dr. Brown performs a comprehensive evaluation that includes:

A detailed history of the accident, the forces involved, your immediate and subsequent symptoms, and any prior conditions that may be relevant. This history is carefully documented in your clinical record.


Orthopedic testing to identify the specific structures that are injured, the degree of injury, and how the injury is affecting the function of the spine and surrounding soft tissue.

Neurological testing to identify any nerve involvement, including assessment of reflexes, sensation, and muscle strength at each level of the spine and in the extremities.


Postural and spinal alignment assessment to identify misalignment and compensatory patterns that develop in response to soft tissue injury.


Digital x-rays when clinically indicated to evaluate bony alignment, joint spacing, and any pre-existing degenerative changes that may be relevant to the injury picture.


When MRI or CT imaging is indicated to evaluate disc injuries or other soft tissue findings more precisely, Dr. Brown will coordinate the appropriate referral.


How We Treat Soft Tissue Injuries


Treatment for soft tissue injuries at Brown Chiropractic Center is targeted at the specific findings of your examination rather than following a generic protocol. The combination of treatments used depends on which structures are injured, the severity of the injury, and how you are responding to care.

Cox Flexion Distraction provides gentle, targeted decompression specifically for disc injuries and nerve root compression, using a rhythmic distraction motion to reduce intradiscal pressure and relieve nerve pain.


Soft Tissue Injuries and Your Personal Injury Case


Soft tissue injuries occupy a complicated position in personal injury cases precisely because they are not visible on standard imaging. Insurance companies frequently use the absence of positive x-ray findings to argue that injuries are minor or nonexistent. A thorough chiropractic examination that documents the clinical signs of soft tissue injury, the orthopedic and neurological findings, the postural assessment, and the ongoing treatment record creates the medical evidence base that counters that argument.


Documentation that begins from your first visit after the accident and continues consistently throughout your treatment tells the complete story of your injuries and your recovery. That story is what your attorney works with, and the more complete and well-documented it is, the stronger your case.


This is one of the reasons I emphasize getting evaluated as soon as possible after an accident. Every day that passes before treatment begins is a day without documentation, and a day that the insurance company can point to as evidence that you were not seriously hurt.
